Whiplash Associated Disorder (WAD) & Neck Trauma Recovery

Whiplash associated disorder, the cervical spine and soft tissue injuries that result from the rapid acceleration-deceleration mechanism of a motor vehicle accident, a sporting collision, or a fall, is one of the conditions most vulnerable to poor acute management producing chronic disability. The research is clear: early active management, guided by a physiotherapist with specific expertise in WAD, produces significantly better long-term outcomes than either prolonged rest or passive treatment. The cervical spine needs to move. The muscles need to be exercised. The neural system needs to be progressively desensitised through controlled movement rather than protected into hypersensitivity.

At this practice, WAD management begins within the first week of injury where possible. Assessment grades the severity of the injury using established WAD classification criteria, guides the appropriate level of early movement, and establishes a baseline against which progress is tracked. Treatment combines gentle cervical mobilisation appropriate to the acute stage, progressive exercise for the deep cervical flexors and thoracic extensors, education about the biology and prognosis of WAD, and (where the injury has generated central sensitisation and persistent pain) an integrated approach addressing both the mechanical and the neurological components of the condition.
