Spinal injury rehabilitation, covering lumbar disc pathology, facet joint injuries, spondylolysis and spondylolisthesis, and the core stability deficits that follow spinal injury or surgery, requires a precise, staged approach that distinguishes between the acute phase, where the priority is reducing loading and pain, and the rehabilitation phase, where the priority is rebuilding the deep spinal stabilising system that protects the spine under load.

Core stability rehabilitation at this practice is built around the evidence for the specific activation timing and recruitment patterns of the deep spinal stabilisers (the multifidus and transversus abdominis) that are consistently disrupted by spinal pain and injury, and that do not automatically recover with pain resolution. Rehabilitation begins with specific low-load activation work for these deep stabilisers, progresses to integrated functional movement with spinal stability maintained under increasing load, and advances to sport-specific or occupational loading that replicates the demands the patient needs to return to. The programme is supervised and progressed at each session, not handed over as a home exercise sheet and monitored at monthly reviews.
