Return-to-play clearance is a decision, and the quality of that decision is the single biggest factor in whether an athlete returns once or returns repeatedly.
Most amateur athletes are cleared on two things: elapsed time and how the injury feels. Both are poor predictors. Tissue healing timelines tell you when a structure can tolerate load, not when the athlete has recovered the strength, symmetry, capacity and movement quality that competition demands. The gap between those two states is where reinjury happens, and in young footballers after ACL reconstruction it is why roughly one in four go on to retear.
Clearance here is criteria-based. Depending on the injury and the sport, the battery covers strength measured objectively and compared side to side, limb symmetry index on hop and jump testing, endurance and repeated-effort capacity, movement quality under fatigue, and demonstrated tolerance of sport-specific loading. The numbers are recorded, tracked across the programme, and retested at milestones.
You receive a written return-to-play report at discharge setting out what was tested, what you achieved, and anything that still needs maintaining. It is a document you can hand to a coach, a club or a school.
