The sharp, stabbing heel pain with the first steps of the morning, easing after a few minutes of walking, is a clinical hallmark, and it is a signal that the plantar fascia has been repeatedly loaded beyond its tissue capacity, generating microtears at the calcaneal insertion. Management requires an understanding of foot biomechanics, calf and intrinsic foot muscle strength, footwear and orthotic considerations, and the progressive loading protocol that drives plantar fascia remodelling. At this practice, management of plantar fasciitis does not begin and end with a calf stretch. It begins with a biomechanical assessment of the entire lower limb loading chain and ends with a return to full running capacity.
